This Privacy Policy explains how Peak Dental Care (“we”, “us”, “the practice”) handles information collected through peakdentalcare.com. It covers the website only.
Information you give us as a patient — during treatment, on intake paperwork, or in your dental record — is protected health information governed by HIPAA and described in our Notice of Privacy Practices. Where website information and health information overlap, we explain that below.
1. Who we are
Peak Dental Care
7920 S. University Blvd., Suite 200, Centennial, CO 80122
Phone: 720-489-0797 · Email: info@peakdentalcare.com
2. Information we collect
2.1 Information you submit
There is one form on this website, on our Contact page. It asks for:
- First and last name
- Phone number
- Email address
- Preferred method of contact
- What you need (for example, requesting or rescheduling an appointment)
- The service you are interested in (for example, general and family dentistry, pediatric dentistry, emergency dental care, cosmetic dentistry, Invisalign, restorative dentistry)
- An optional free-text message
- Your agreement to be contacted by text message and/or email
Please do not send detailed health information through this form. It is intended for scheduling and general enquiries. If you need to discuss symptoms, medical history, medications or treatment, please call us instead.
Because a submission combines your identity with the dental service you are asking about, we treat completed enquiries as confidential and handle them with the same care as patient information.
2.2 Information collected automatically
When you browse the site, our host and security software record standard technical data: your IP address, browser and device type, the pages you view, the date and time, and the page that referred you. Analytics and advertising-related cookies are described in section 4.
2.3 Information we do not collect here
This website has no patient portal, no user accounts, no registration and no login for visitors. Comments are disabled across the site. We do not sell anything online and do not process payments through this website.
3. How we use information
- To respond to your enquiry and schedule, confirm or reschedule appointments
- To contact you by your chosen method, where you have consented
- To operate, secure and maintain the website
- To understand which pages and services visitors find useful, so we can improve the site
- To meet our legal, professional and record-keeping obligations

Your browser settings can block or delete cookies. You can opt out of Google Analytics using Google’s browser add-on. Blocking cookies will not stop you using the site or contacting us.
5. Who we share information with
We do not sell your personal information, and we do not share it for anyone else’s marketing.
We use the following service providers, who handle information on our behalf:
- SiteGround — website hosting. Form submissions are stored in the website’s database and server logs are retained on their systems.
- Resend — delivers the notification email generated when you submit the form. The contents of your submission pass through this service.
- Microsoft 365 — our practice email.
- Wordfence — website security. Records IP addresses and request details to block attacks.
- Rocket Small Business Services — our website agency, which maintains the site.
We may also disclose information where the law requires it, to protect the safety or rights of our patients, staff or practice, or in connection with a sale or transfer of the practice.
6. Links to other services
Our Patient Forms link takes you to a secure form hosted by Henry Schein One (mydentistlink.com). Information you enter there is collected by that service on our behalf and is not governed by this policy. We also link to Google Maps, Facebook, Instagram and Yelp. Those companies have their own privacy policies.
7. How long we keep information
Enquiries submitted through the contact form are stored in the website database and in our email. Submissions currently held date back to March 2024. We retain them while they remain useful for patient service and to meet our record-keeping obligations. If you would like your website enquiry deleted, contact us and we will remove it unless we are required to keep it.
Information in your dental record is kept for the period required by Colorado law and professional standards, and is governed by our Notice of Privacy Practices rather than this policy.
8. Text messages and email
If you tick the consent box on our form, you agree that we may contact you by text message and/or email about your enquiry and your care. Message frequency varies. Message and data rates may apply. You can stop text messages at any time by replying STOP, or ask us to stop contacting you by calling the office. Consent to receive texts is not a condition of receiving treatment.
9. Your choices and rights
You may ask us to:
- tell you what information we hold about you from this website;
- correct it if it is wrong;
- delete your website enquiry;
- stop contacting you by text, email or phone.
Colorado residents have additional rights under the Colorado Privacy Act, including the right to access, correct, delete and obtain a copy of personal data, and to opt out of targeted advertising, the sale of personal data and certain profiling. Information held in your dental record carries separate rights under HIPAA, described in our Notice of Privacy Practices.
To make a request, contact us using the details in section 11. We will verify your identity before acting on it.
10. Children
We treat children as patients, and a parent or guardian may contact us on their behalf. This website is not directed at children, and we do not knowingly collect information online directly from a child under 13. If you believe a child has submitted information through the site, please contact us and we will delete it.
11. Security
The website is served over an encrypted HTTPS connection, kept up to date, and protected by a firewall and malware scanning. No website can be guaranteed completely secure, which is why we ask you not to send detailed health information through the contact form.
